Add entries to the screen dynamically with one button

I have a service registration form, but I wanted to have the option of registering a service that in its display has a selector, at first I thought about using the form an option where I clicked on a button and added more inputs to add each type to this service but I couldn’t find a way to add more inputs to the screen according to the button clicked, I wanted help with this logic, how to do this in a practical way in bubble
