Auto dynamic rows addition

Hello Everyone
i need assitance to this issue,

i have seen some couple of related threads but yet not very clear and still getting error

I have a user clicking “Create New Form” takes them to the form page, I create a new form with 1 row and then let the user create as many rows as they want. That if the user clicks “Add Row” then another row will populate.
