How to create a text annotation system?

I’m trying to create something a little tricky: to give users the ability to annotate a text. Examples:

Google Docs:

Medium:

However, I can’t find a way to create a workflow or trigger an actions by selecting part of the text. Does anyone have any idea or workarounds?
