Need a freelancer for a custom order form

Hello,

I need a web app that turns this google sheet into an order form. There are input restrictions in most columns. I do not need any back end development, as I will only need the form to send the final table in any format (pdf, txt, xls) to a specified email for manual revision. The form is this one:

At the end it would total the amount of units, and ask the user if he wants to receive a copy to his email
