Thing being deleted automatically after creation, but deletion is not in workflow

Hi, in my app I have a Thing called ‘Tax Rate’ which consists of a Rate (Number) and a Name (text).

I have 2 inputs for creating a Tax Rate. Input Name which is Text, and Input Rate which is Percentage.

Upon pressing button ‘Create’ the workflow is set to create Tax Rate then display it in a group to see it’s Name and Rate.

However, after it has been created and displayed, it seems to get wiped from the database and I can’t figure out why. Thank you.
