Workflow missed step or can't find? Reading service logs

Hi,

I noticed that for this workflow on the left, for some reason, when it was making a change, it could not find the object to make a change. While on the right, it was able to find the object. The object that it is updating existed already. This workflow is the same steps but just different user. I do not know why one user was able to find one and one user was not able to find one.

Also, for service log, what is button A? Why does sometimes the actual button name is correct and sometimes it isn’t there. If i click the Button A, it will redirect me to the workflow where the button has an actual name.
