Why are my input fields not working?

I have a form with 5-6 input boxes, 2 radio button, 2 set of 2 check-boxes etc.

Some of the elements are not responding in the preview. Some input boxes are not editable, some check-boxes are not working, radio button works and stops working sometimes.

Has anyone come across this issue before? I have tried deleting and creating new elements. The issue still persists.

Any ideas?
