Date AM/PM in label

Need help

I am looking to format a label with the following data coming from a date time field.
m/dd/yy H:MM

I want to have it in AM/PM format and can’t figure out what needs to be at the end of the custom format.

I’m sure this is easy, but I could not find it anywhere. I appreciate the help!
