Help with Basic Input Data and Search for data function

Hello All,

I am trying to create a simple site that has a home page, a search page, and an input page. Basically I have created a home page with 2 buttons, 1 to search, 1 to input. What I want to do is be able to input some data on the input page and be able to search for the data on the search page.

I have all of this laid out the way I would like it and have tried to set the data part of it up and there is something that I am missing and I cannot figure out what it is.

Any and all help is greatly appreciated.
