Data in list not added even though it is shown to be added in logs

I have 2 lists in which I am trying to add data via Workflow on a list. The logs shows that the workflow has run successfully and added 2 entries in the 2 lists. However, I do not find 2 entries in the DB and the first one is always missing. Only the second is shown in DB for both the lists.

is it a bug?
