Help in creating pdf or jpg of filled form

Hello everyone,
So I built this order form for my clients:


I need to make the workflow so that when the user clicks the button, I get a screenshot of the webpage with all of the information the user inputed. I have tried:
Convertio
Html2PDFRocket
PDFconjurer
Screenshotlayer
Have not been able to get any of them to work. Any help would be greatly appreciated

Have you tried the element 2 image plugin by codurly?

We have a tutorial for it at nocodify.com, may help you…
