Are you using privacy controls at all in your app? Once you set a data privacy role you will need to make sure those fields are visible to the current user based on the permissions you set.
When you enable debug_mode, are there any errors that appear when you inspect the input fields?
Edit: I’d suggest enabling auto-binding and setting permissions for auto-binding in the privacy data role panel.
