Making changes on a thing. (Backend workflow)

Hello everyone, is there anyone experience the same scenario when adding a thing to the list of thing inside another thing. It seems that the workflow can’t follow or can’t consistently proceed in the steps. For example in this scenario where I want to create multiple Feedback items and add this inside the review. I created 5 feedbacks but only 3 or 2 items are added inside the review.
