Single record for multiple data entry points

Hi Guys, So i am new to Bubble and this might seem like a stupid question so any help would be much appreciated.

The system I am designing requires a user to enter his/her name, and then to make a selection from 4 choices. Although I have created workflows for this, the name gets entered as a record, while one of the choices made gets entered as a different record. How do I ensure that the name and choice is entered into a single record.

Are you able to show some screenshots of the workflows? Unsure where things are being saved without seeing it.

Or possibly a link to the app editor (will need to make the app public to do this)
