How to create a CRUD Form and list immediately the entries in a grid list style format

Hi guys,

Im just a starter/beginner in Bubble and for the purpose of learning processes, I want to create a CRUD form having 5 input fields with a Save/Submit Button. Also, below it with a grid list that shows all the saved entries from the form.

Anyone here does have a sample for a CRUD FORM alike?
