I have this weird issue with my website that images I upload from option set keep disappearing from time to time, it’s happened more than 3 times and I keep re-uploading.
Can anybody please point out what could be the issue and how this can be fixed?

Did you take a look at the register to understand what’s going on? But do the images also disappear from the file manager or are they there and are not readable? Are you uploading images with the same name as images already present in the file manager?
Double check to see that the images are still in the file manager–if they aren’t, there may be a loose workflow somewhere that is deleting them.
You don’t always need to use the ‘delete’ feature to delete an item too. For instance if you have a workflow that is changing a thing, and a data type on that is set to [Data = ‘empty’} it will delete the piece of data in that data type when that workflow is run if that piece of data is not specified to anything
The images are still in the file manager, I double checked. but image/ app graphics still disappear often.
I use option sets for almost all my images and icons, and I’ve actually had this happen to me a couple of times. I just figured I had done something wrong because I never could figure out what happened. I just re-added them to the option set. Maybe I did do something wrong, but I still don’t know
