When you create a thing (e.g. parent’s child) using the ‘make changes’ workflow action, it creates the new thing (child) but does not link it to the intended field on parent object as seen in example on the page.
I don’t think this is a bug, but more of an intentional design choice. Semantically, this operation should only modify a single object. There’s also an issue when the object was accessed from a list; it might be a bit difficult to assign that relationship (e.g. Parent group’s Parent object’s ChildrenObjects :item# 4).
