Required input setting doesn't seem to be working

Hi, everyone:

I don’t understand why this is happening. I have a form in which some of the fields are required. I have checked “This input should not be empty” like so:

and formatted the style like so:

But some of those fields (i.e. Last Name) are not showing the error/invalid style like the others:

I’ve tried refreshing the page as well as previewing the page on a second device, but still got the same results. Any thoughts?

You need to actually reference the inputs in some workflow for them to exhibit this behavior. Are you actually referencing them in your create/make changes workflow?
